Giovanni Molet is sent to jail for the murder of two of the Carraro family over a feud over land. While incarcerated, a vendetta is carried out on Molet's brother who is killed by the Carraros. While being transferred to another prison, Molet manages to escape and goes on the run. He holes up in a large villa owned by a newlywed couple and slowly starts to prepare for his revenge.

A psychotic man opens fire in a diner, murdering numerous people before killing himself. The survivors struggle in different ways following this horrendous event: a doctor doubts his own instincts and elects to use an experimental medical procedure on his wife, while a gambler believes he's on a lucky streak. A waitress begins engaging in promiscuous sex, and a young girl whose father is among the dead gains unexpected fame.

In the late 1970s and early 80s, assassinations in Sicily get the attention of Communist deputy, Pio La Torre, who appeals to General Carlo Alberto Dalla Chiesa to become prefect in Palermo and take on the Mafia. Dalla Chiesa approaches the job with the same focus and methods he used in hunting down the Red Brigade.
