As the Naito Shinjuku family’s split conflict deepens into even greater chaos, Reiji Toki (Yoshiyuki Yamaguchi) is defeated after a battle beyond human understanding with Kiyoharu Washio (Kazuyoshi Ozawa). In this dire situation where Reiji’s life hangs in the balance, a tense atmosphere begins to spread even within the faction supporting Taiga Washio (Haruto Ochiai). Tetsutake Yu (Takashi Kuroishi) grows suspicious of former violent crimes detective Shinichi Akiba (Yasukaze Motomiya) acting alone and locks him up. In an alternate version of 1949 Japan in which World War II never happened, the Japanese capital of Teito is home to both an ultra rich upper class and the dirt poor masses. The city is thrown into a state of panic when a phantom thief called “The Kaijin (Fiend) with 20 Faces” (K-20 for short) begins to use his mysterious abilities to steal from the rich and give to the poor. One day a circus acrobat named Heikichi Endo (Takeshi Kaneshiro) is framed for K-20’s crimes and becomes determined to clear his name. He teams up with K-20’s next target, a wealthy duchess named Yoko Hashiba (Takako Matsu) and her detective fiancé (Toru Nakamura), to take K-20 down once and for all.

Shibuya, Tokyo, is bustling with the Halloween season. A wedding is being held at Shibuya Hikarie, where Detective Miwako Sato of the Metropolitan Police Department is dressed in a wedding dress. While Conan and the other invited guests are watching, an assailant suddenly bursts in, and Detective Wataru Takagi, who was trying to protect Sato, is injured. Takagi survived and the situation was settled, but in Sato's eyes, the image of the grim reaper that she had seen when Detective Matsuda, the man she had been in love with, had been killed in a series of bombings three years ago, overlapped with Takagi's.
